Дуоденальные неязвенные кровотечения: причины, диагностика и лечение

Желудочно-кишечные кровотечения остаются актуальной проблемой современной медицины. Заболевания двенадцатиперстной кишки неязвенной этиологии, осложненные кровотечением, требуют дальнейшего изучения, совершенствования тактики диагностики и лечения в ургентных условиях. Авторами проведен анализ современной отечественной и зарубежной литература по данной тематике. Большинство сообщений в литературе, обычно публикуемых в виде описаний случаев заболевания, содержат небольшое число пациентов и не связаны с таким осложнением, как кровотечение. Приведен собственный опыт лечения 30 пациентов с дуоденальными кровотечениями неязвенной этиологии. Наиболее частыми причинами дуоденальных неязвенных кровотечений являются: полипы двенадцатиперстной кишки, доброкачественные опухоли двенадцатиперстной кишки, гастроинтестинальные стромальные опухоли, дивертикулы двенадцатиперстной кишки, дуоденальная дистрофия вследствие groove-панкреатита, эктопия поджелудочной железы в двенадцатиперстную кишку, истинные аневризмы парапанкреатических артерий. Пациентам проводились как малоинвазивные вмешательства, так и различные виды открытых хирургических операций, направленных на достижение гемостаза и радикальную коррекцию имеющейся патологии.

Duodenal non-ulcerative bleeding: causes, diagnostics and treatment

Gastrointestinal bleeding remains the current interest in medical practice. Diseases of the duodenum of non-ulcerative etiology, complicated by bleeding, require further study, improvement of survey and treatment in urgent conditions. Authors conducted the rewiew of contemporary data in the literature, regarding on this pathology. Most reports in the literature, usually published as case reports, contain a small number of patients and do not concern bleeding. Authors report their own experience on treatment of 30 patients with duodenal bleeding of non-ulcerative etiology. The most common causes of duodenal non-ulcerative bleeding are duodenal polyps, benign duodenal tumors, gastrointestinal stromal tumors, duodenal diverticula, duodenal dystrophy due to groove-pancreatitis, pancreatic heterotopy in the duodenal wall, true aneurysms of the parapancreatic arteries. Various types of open surgical operations, as well as mini-invasive interventions, were performed to achieve hemostasis and provide radical correction of the existing pathology.
