We carried out complex clinical and laboratory examination of 718 women that underwent in vitro fertilization program (IVF) in “Center of Reproductive Medicine” (Minsk, Belarus) in 2014–2017; we divided the cohort into two groups depending on controlled ovarian stimulation protocol. The research was aimed at evaluation of index of ovarian reserve and response to controlled ovarian stimulation and its role in prediction of ovarian hyperstimulation syndrome in women undergoing in vitro fertilization. We established statistically significant difference between the ovarian reserve and response index (ORRI): 3 to 5 times higher in patients with ovarian hyperstimulation syndrome. The main outcome of the research was the establishment of statistically significant correlation (correlation coefficient 0,572) between ORRI and OHSS in women undergoing IVF. We also established cut-off level of ORRI (1.52) associated with elevated risks of OHSS development with high sensitivity (Se – 95.2 %) and specificity (Sp – 90.4 %). Therefore, ORRI should be regarded as a valuable prognostic marker of ovarian hyperstimulation syndrome in women undergoing IVF and should be recommended for implementation in clinical practice.
