The results of a retrospective analysis of patients case histories with acute appendicitis after kidney transplantation from deceased donors are shown. The clinical data of the patients, the characteristics of the preoperative and postoperative period, the type of operation are presented. Patients with kidney allograft didn’t have atypical clinical manifestations of acute appendicitis. Acute appendicitis after kidney transplantation from a deceased donor is rare. When a patient receives a kidney transplant from a deceased donor with suspected acute appendicitis, a set of examinations is required to exclude the pathology of the transplanted kidney. In the presence of renal allograft in the right iliac region, the displacement of the cecum with the Appendix should be taken into account, which can localize the symptoms of appendicitis in other anatomical areas. In the cases considered, the course of acute appendicitis was typical, which allowed surgeons already in the emergency Department to diagnose upon admission – acute appendicitis “?”. Any type of surgery in acute appendicitis does not affect the function of the renal transplant.
