Данный клинический обзор посвящен болезни Уиппла. Приведены данные по ее эпидемиологии, рассмотрены современные представления о патогенезе данной редкой системной патологии, охарактеризована клиническая симптоматика болезни. Подчеркивается частота суставного синдрома при болезни Уиппла, и, в особенности, его возможное доминирование в клинической картине, но без проявлений со стороны желудочно-кишечного тракта. Детально описаны диагностические возможности лабораторных и инструментальных методов исследования, приведены алгоритмы верификации диагноза, в том числе при наличии артрита. Рассмотрена концепция лечения и возможные варианты современной этиотропной терапии. В конце обзора описан интересный клинический случай наблюдения пациента с длительным рецидивирующим активным полиартритом и поздней диагностикой болезни Уиппла, приведшей, тем не менее, к положительному эффекту соответствующего лечения.

Whipple’s disease

The presented clinical review covers the Whipple’s disease. The data on the epidemiology and the current knowledge on the pathogenesis of this rare system pathology are considered, clinical manifestations and presentations are described. The frequency of joint involvement in Whipple’s disease is emphasized and specifically the cases of Whipple’s disease with the dominance of arthritis presentation and in the absence of any gastrointestinal symptoms. Diagnostic value of laboratory and instrumental methods is described in detail, algorithms of diagnosis verification are given, taking into account the cases of arthritis manifestations. Treatment concept and possible options of current etiotropic treatment are considered. In addition an interesting case of patient history with long-term intermittent active polyarthritis and late diagnosis of Whipple’s disease is described, though resulted in successful outcome of adequate treatment.
